In this stanza, Vicente Fernández is talking to his own heart, not his sweetheart or lover. ¿Qué tal? alone is used as shorthand for ¿Qué tal va?, “how does it go?” or “how is it going?”, but here the verb is sentirse (to feel, reflexive). He is asking his own heart how it feels to endure the consequences of his actions.

Pegar means “to hit”, translated as “to punch” in the main translation to keep the same feeling.
